Care Plus Program

The Care Plus Program and System of Care Data Integration System (SOCDIS) exemplify program design and administration for vulnerable populations and the complexities of operational and data systems integration.

In 2019, Orange County's Point-In-Time Count identified 6,860 homeless individuals, including 3,961 unsheltered. The System of Care Data Integration System (SOCDIS) integrated nine County databases, identifying "highest utilizers"—homeless individuals appearing in three or more databases. The Care Plus Program was developed to coordinate care for these individuals through Virtual and Intensive Care Coordination.
