Challenge

In 2017, the County of Orange Board of Supervisors recognized the need for a data integration platform to connect behavioral health, public health, housing, HMIS, ArcGIS, CalWIN, and jail booking systems to coordinate care for the homeless. Once the platform was operational, the Care Plus Program was launched to manage the County's most chronically homeless individuals within the system of care.

Solution

Developed performance metrics and secured approval from all 9 County departments involved in the Care Plus Program.

Created dashboards to track performance, reviewed monthly.

Addressed concerns around sharing PII, health, criminal justice, and social services data by forming a data governance work group led by the County's Chief Privacy Officer. Instituted an executive-level steering committee for policy decisions.

Established an inter-departmental, multidisciplinary team with clear goals, agendas, and regular meetings to case conference the 85 most chronically homeless individuals.
